Care Management as a Service Patient and Financial Outcomes Study
Orb Health partnered with a large FQHC to deliver Chronic Care Management to their ACO member population and achieved tremendous results. They expanded their Care Management as a Service into scaling patient access and Transitions of Care servicing and saw significant additive results.
In the first 9 months of the program, a 54% decrease in ED visits for patients with 6 or more conditions was observed with 54% reduction in emergency department utilization, resulting in an estimated $6.2 mm in net Medicare savings for every 1,000 patients.
Once they expanded to include contact center services, the studied provider realized a 22% increase in patient outpatient scheduling, 100% schedule utilization within two weeks of go live, 26% affected patient cost reduction with a 14% increase in coding and a care gap closure rate of 70%.
